CHILDHOOD ABUSE IN LATE-LIFE DEPRESSION Results from Results from Netherlands Study of Depression in Older persons (NESDO). Hannie C. Comijs, Eric van Exel, Roos C. van der Mast, Anna Paauw, Richard Oude Voshaar, Max L. Stek EMGO-Institute, VU University Medical Center, dep Psychiatry, Amsterdam, The Netherlands AimsInstruments To examine:  the association between childhood abuse and late-life depression  which clinical characteristics and comorbidities play a role in this association. Results INFO Design  Composite International Diagnostic Interview (CIDI) for diagnosis of depressive disorders  Childhood abuse, including psychological, physical and sexual abuse as well as emotional neglect, was assessed using a structured inventory  378 depressed (major depression, dysthymia or minor depression according to DSM-IV criteria),  132 non-depressed persons,  Age range: 60 through 93 years Acknowledgement Conclusion Statistics Multivariate multinominal and logistic regression analyses Early onset (<40 yrs) n=121 Middle age onset (40-60 yrs) n=120 Late onset (>60 yrs) n=124 OR95%CIOR95%CIOR95%CI Emotional neglect Psychological abuse Physical abuse Sexual abuse Any abuse Results of multinominal regression analyse, with type of depression onset as outcome and non-depressed adults as reference group (adjusted for age, sex and level of education) Early onset (<40 yrs) n=121 Middle age onset (40-60 yrs) n=120 Late onset (>60 yrs) n=124 OR95%CIOR95%CIOR95%CI Age (in years) Female Years of education Cognitive function (MMSE) # Chronic diseases Results of multivariate logistic regression analyses of the association between age, cognitive functioning and number of chronic diseases, and childhood abuse within categories of onset of depression The infrastructure for NESDO is funded through the Fonds NutsOhra, Stichting tot Steun VCVGZ, NARSAD The Brain and Behaviour Research Fund, and the participating universities and mental health care organizations (VU University Medical Center, Leiden University Medical Center, University Medical Center Groningen, Radboud University Nijmegen Medical Center, and GGZ inGeest, GGNet, GGZ Nijmegen, GGZ Rivierduinen, Lentis, and Parnassia). Comijs et al. The Netherlands study of depression in older persons (NESDO); a prospective cohort study. BMC. Res. Notes 2011, 4(1), 524. Comijs HC, et al. Childhood abuse in late-life depression. J of Affective Disorders, in press Childhood abuse is lifelong and strongly related to depression, even in case of late-onset depression. This may suggest that the psychological wellbeing persons were able to maintain during their lives, may become disturbed later in life. Limitations Age of depression onset and childhood abuse was asked retrospectively, which may have biased the results The depressed persons did not differ from the non-depressed controls with respect to mean age (70.7 versus 70.1 years) and sex (66% versus 61.4% female), but had a lower level of education (10.4 years versus 12.5 years of education; p<0.001). Depressed persons reported significantly more often childhood abuse (53.3%) than non-depressed controls (15.9%); emotional neglect: 41.5%, psychological abuse: 26.3%, sexual abuse: 22.6% and physical abuse:16% In depressed persons, all types of abuse were associated with more severe depression, a younger age of onset, more depressive episodes, higher levels of neuroticism, lower sense of mastery, and higher number of chronic diseases. In addition, psychological abuse and physical abuse were also associated with more anxiety symptoms and co morbidity of anxiety disorders. Only physical abuse was associated with lower cognitive function. Childhood abuse was not associated with lifetime suicide attempts and apathy symptoms in late-life depression.
