Reporter Training for High School RIO TM

The link in the weekly reminder will bring you to the login page After returning your participation form, you will receive an containing your username (your ) and your password to log into the site. Your default password will be You will be prompted to change your password the first time you log in.

After you log in for the first time, you will be asked to change your password. Once you have done so, click “Set password” You will be redirected to the exposure/injury report dashboard

After you have changed your password, you are ready to navigate the system using the center tool bar under the High School RIO™ icon. However, if your contact information changes during the course of the study, you can update it. To update your address, click on “Edit my profile” on the exposure/injury dashboard screen under “Sign Out”. Then click on personal settings.

Make sure the week you click corresponds with the week dates you are reporting for. To begin reporting study data, click on “Exposure Report Form” for the week you would like to add an “Exposure Report”. The system will not allow you to complete an exposure report for weeks in the future. The exposure audit flags allow you to visually see which weeks have a completed exposure report form. The flag will be red or yellow if further action is needed. Click on the red or yellow flag to find out more information about what needs to be done. A green flag will appear when the exposure report form is complete.

Important definitions of injury and exposure are provided at the beginning of the weekly exposure report

At any time you can click save to save as you go, save & exit to go to the exposure/injury report dashboard, or exit without saving to leave this page. For each sport, select whether or not the sport is in season. If the sport is in season, enter the number of athlete-practices and athlete-competitions that took place in the past week in the boxes below the appropriate sport. Also enter the number of injuries that occurred.

After completing the exposure report, an injury report form will automatically appear on the exposure/injury dashboard for each injury reported on the weekly exposure report. Click the “edit” button to enter the injury report to provide required data. Click “delete” to remove the injury report and “summary” for an overview of the injury data entered. To add additional injury reports, click “Add Injury”.

The number of injuries reported in the weekly exposure report should match the number of injury report forms submitted for the week. If you become aware of additional injuries, please update the weekly exposure report with the new number of injuries. The completed Week 1 exposure report can be modified by clicking “Exposure Report Form”

You must use this tab to identify the injured student. To make reporting easier, we have provided the option for you to enter the student’s name (so you don’t have to remember ID codes for injured students throughout the season/school year). However, the researchers will only see the computer generated ID code. Student names entered here are for your use only – they are not captured in the study dataset. Here you have 3 options. Select a pre-populated student name from the existing list, create a new student, or have the system generate a de-identified student code with no accompanying name. You must do one of the 3 to access the rest of the tabs.

This is the “Athlete Demographics” Tab. Please enter data requested on this tab to the best of your ability. You will receive an error message if data is entered in the incorrect format. The sport the athlete was participating in will be automatically populated from the weekly exposure report.

This is the “Injury Info” tab. You must enter data requested on this tab to participate in the study. Please continue to answer the questions to the best of your ability You may notice imbedded “skip patterns” as you complete this tab. For example, if “mouth/teeth” is selected, a question regarding mouthguard usage will appear below this question. Similarly, if knee or ankle is selected, a question regarding type of knee or ankle injury will appear below

This is the “Injury Event Info” tab. You must enter data in this tab to the best of your ability to participate in the study. When choosing “Other” in any of the dropdown menus or checkboxes, a text box will appear for you to type in a brief description.

This is the “Sport-Specific Info” tab. You need to enter data in this tab to to the best of your ability to participate in the study. Click save as you progress through the report. When you are finished, click save and exit.

The EMR tab allows you to track an athlete’s current participation status, to enter SOAP notes, and to document assessments, treatments, and outcomes This is the “Medical Notes” or “EMR Tab”. This tab has been made available to you for your clinical use as a token of our appreciation for your participation in the study. You do not have to enter any data on this tab to participate in the study. If you choose to utilize this clinical tool the researchers will never see any of the information you enter on this tab.

The reported injury is shown on the main page. To modify this report, click “Edit”. If the injury report form needs to be deleted (i.e., you report an injury for a player you thought would be out at least one day but the athlete returned to practice the next day so they no longer meet the definition of injury, the same injury was reported twice, etc.) it can be deleted by clicking “delete”. This generates a “Student Report”. The information used to generate this report comes from the injury tab and the EMR tab of the injury report. This report can be printed out (to send home to parents, etc) if you choose.

There are also auditing flags for injury reports. Click on the flag to find out more information. Green flag: Injury report form is complete. Yellow flag: There are several “primary questions” that we would like to have completed for each injury. If you have not answered very many of these “primary questions”, you will see a yellow flag. However, please note that these questions are not required. Complete the “primary questions” as well as all other questions to the best of your ability. Blue flag: You have completed most but not all of the “primary questions”. Red flag: You have completed none of the “primary questions”.

Important Reminders Complete an exposure report for every week that a sport is in season, even if your teams are inactive (if a team is inactive, simply report “0”, “0”, and “0” for athlete-practices, athlete-competitions, and number of injuries). If no sport that you report for is in season, please still log in and select ‘No’ for sports that you are reporting for that are not in season and ‘N/A’ for sports that you are not reporting for.
