Obtaining a Passport  Obtaining a Passport Obtaining a Passport  Passports must be obtained and copied for turn-in by March meeting  If you already have a Passport, check the expiration date  Passport Photos  Students hold on to their passports while in transit, upon check in at the hotel, chaperones will collect them and put them in a safe.  Students MUST provide a copy by 1/28

Passport Requirements 1st Time Applicants

1.Fill Out Form DS-11: Application For A U.S. Passport *need Social Security number **Online or by hand 2.Submit Completed Form DS-11 *In Person. See website for places to do this 3.Submit Evidence of U.S. Citizenship*birth certificate 4.Present Identification 5.Submit a Photocopy of the Identification Document(s) Presented (Step 4) 6.Pay the Applicable Fee*minors $95 + ($25 fees)= $120 7.Provide One Passport Photo *Riteaid, CVS, etc Passport Procedures…

Confirmation of Names  You must logon to Eftours.com and confirm the traveler’s name by January meeting.

Transfer to Airport on July 25 th  Leaving from CB South High School  Each student pays $30-$40 for bus to airport  Parents pick students up at SOUTH upon return

Forms to Sign  Medical Profile – must be signed and turned in on 1/28/16  Central Bucks School Board Policies #231 regarding Student Travel (read #8 and #12 specifically) and #227 regarding Controlled Substances. These policies must be reviewed and acknowledged with initials by 1/28/16  EF Alcohol Policy form signed by 1/28/16  Group Leader South – Srta. Gray Building Contacts with questions South – Mme. Chemin or M. Valois West - Sra. Divita East – Mme. Fichtman or Sra. Poblano
