 Funds Available through State General Revenue  Getting Everyone on Board – F4A  Testing Software  Software Selection  Workgroup Formed  Final Implementation

 All Community Living Program (CLP) clients come through the single-point of entry, the Aging Resource Center  Entered in Refernet and coded as “Transition to Home” clients  Refernet interfaces with our statewide client management system (CIRTS)

 Support of the Legislature and the Governor through Dedicated Funding  Collaboration of the 11 Area Agencies on Aging – One Common Goal  Clearly Defined Roles with Focus on Standardization  Ongoing Commitment through the Information and Referral Workgroup
