By collecting the following data that will be entered on the Minnesota Department of Health’s Surveillance Form: Partner’s Name (first, last and nicknames) Partner’s address with city and state if available Partner’s phone numbers (mobile, home, work etc.) Partner’s race age, sex, and DOB Date of last exposure Partner’s physical description (height, weight, eye color, hair style/color. Any additional locating or identifying information that the patient may have

When you have a positive result, you are required to notify MDH two ways – EvaluationWeb and HIV Case Report:
HIV is a reportable disease in Minnesota.
Enter the positive test in EvaluationWeb, and make note of the last six digits of the auto-assigned unique Form ID number. This is the first line of the test form you see when you open a new form for data entry.
Choose “Yes, Show HIV Test 2” if a second rapid or confirmatory blood draw test is done during the same session.

Keeping in mind: 4 th Gen increased processing time If patient is identified as a confirmed positive, consider the time it takes to link and verify HIV primary care NOTE: It is important to target high risk individuals.
