CCWC October 1, 2008

Overview  What is Public Health?  CCWC Endorsed Initiatives Health Improvement Plan (HIP) Infant Mortality Reduction Coalition  What’s on the horizon?

Public Health  Not just medical care  Everything we do to assure the conditions in which people can be healthy  Governmental Public Health Core Functions Assessment Assurance Policy Development

Public Health Programs and Services  Surveillance MDSS Morbidity/Mortality Community Surveys Epidemiology  Emergency Preparedness and Response  Health Promotion  Environmental Health  Family Health WIC MIHP Hearing & Vision CSHCS  Disease Control Immunizations Sexually Transmitted Infections/HIV Testing TB Control

Health Improvement Plan Since 1995 PH has led this countywide partnership. HIP’s purpose is to facilitate the design and implementation of data driven collaborative community-based disease prevention and health protection strategies that promote the health of everyone in the county. Partners include: health systems and health care providers, community agencies, universities, county residents

Data Driven  HIP Survey every 5 years—BRFS plus  Morbidity and mortality  2020 Objectives Focus areas: physical activity, healthy eating, tobacco reduction and depression

Searchable, database, online, generates bar charts and geographical/mapped data by race, age, income, health status, gender ; new non-survey data will be available online soon

Collaborative Strategies  Health disparities, inequities  Evidence based practices  Partnerships and networking  Policy change at the policy and environmental level  Social-ecological model

Family Organizations Individual Neighborhoods Policy and Built Environment Schools Churches Workplaces The Social Ecological Model Media Peers Healthcare

Infant Mortality Reduction Coalition Update  Higher rates of infant mortality and low birthweight for Black infants  Healthy Michigan Fund  Focus groups reveal racism in prenatal care  Undiagnosed infections  Dental health  Access to prenatal care  Increasingly prescriptive  Only allowable costs are interconceptual care  No longer able to use funding to support coalition  Local funding reductions affect our ability to continue to convene coaltion, rolling into HIP

On the horizon…  Suicide prevention plan  Expansion of data on social factors that are underlying cause of persistent health disparities  MiPHY  Gearing up for 2010 Survey  Is there a role PH can play in helping CCWC collect and maintain data to support our work together?
