Strategic Goals Ensure Housing Placement of Chronically Homeless Individuals into Decent and Affordable Permanent Housing as Quickly as Possible Help Newly Housed Individuals Achieve Housing Stability by Assuring Access to the Appropriate Level of Supportive Services Collectively Advocate for more Permanent Housing, Funding and Community Support for our Mission Build Capacity for Leadership, Partnership, and Service Delivery

Outcomes As of July 31, 2014, the Partnership has moved 376 Chronically Homeless Individuals into Permanent Housing 145 of those Individuals were Housed using a Housing First Model through Pathways to Housing PA in Subsidized Market Rent Apartments throughout the City 90% of these Individuals in Permanent Housing have achieved Housing Stability for 1 Year or Longer
