New Policies 1. Set Program Year  July 1 st -June 30 th 2. Application  1 application per client a year  Updated every time client receives services

New Claim Policies  Max 12 /year & 1/month  Claiming one month at a time  All claims are due by the 18 th of the month  Cover Page for every line item you claim  Copies of documentation

Claims Process  Fill out normal claim voucher  Fill out corresponding excel sheet for line items claiming  Attach documentation in order under corresponding excel sheet  Fax or claim to me by the 18 th  Budget Limitations

Claims/Budget Modifications Budget limitation formulas  Administration/Total Budget Request=not exceed 7%  Housing Ratio= (ST+LT+SS)/total budget request=not be less than 60%  LT rental assistance program delivery/total amount of HOPWA rental assistance=not exceed 3%  Short Term Program Delivery/total amount of HOPWA short term assistance=not exceed 3%

Claims *Do not need the narrative anymore *Do not include documentation for things you are not claiming Claim Checklist ___ Completed Excel Sheet for every claim line item ___Copy of Supportive documentation for all line items ___Excel sheet and documentation in corresponding order ___Items claimed can only be for one month (the month you are claiming) ___All receipts/bills should have an account number written somewhere showing what account the money was draw from
