TASK TRAINING OUTLINE REVIEW OF Owner USE POLICY REVIEW OF SOP REVIEW OF JSA REVIEW OF PRE-OP FORMS FIELD TASK TRAINING

Vehicle Use Policy This policy applies to the use of all Owner International, Inc. (Owner) Owned equipment, including, but not limited to, the 2005 Polaris Ranger and Wells Cargo Road Force Trailer. This policy shall remain in effect until modified or removed by Owner. Any person causing damage to any Owner owned equipment may be held financially liable if damage results from misuse or carelessness. Equipment is to be stored in a location designated by Owner when not in use. This policy will cover: 2005 Polaris Ranger 2006 Road Force Trailer Polaris Support Equipment

Vehicle Use Policy Road Force Trailer  Trailer is to remain secure at all times. Locks are to be kept in-place and locked at all times when equipment is not being loaded or unloaded. Hitch lock is to be locked and in-place at all times when trailer is not connected to towing vehicle.  Keys to trailer are, at all times, to remain in the possession of the person(s) responsible for utilizing the equipment. Keys shall not be left in towing vehicle or trailer  During periods that the trailer is being used to store the Polaris, the trailer shall be secured utilizing a lock and chain to an immovable object

Vehicle Use Policy Road Force Trailer  Trailer shall only be towed by vehicles equipped with a 2-5/16 inch trailer ball and drop type hitch. Towing vehicle must be equipped and capable of utilizing electric trailer brakes. Safety chains and break-away system must be connected properly to towing vehicle.  Trailer shall not be taken off-road. Trailer shall be pulled near to location and parked on level, hard ground to off-load the Polaris. At no time is trailer to be taken into rough, un-level areas or cross country.  No food or drinks are allowed in trailer. Trailer is for equipment only

Vehicle Use Policy Road Force Trailer  No smoking or tobacco products are allowed in trailer  Trailer is to be swept out periodically to remove dirt and debris from floor. Exterior of trailer is to be washed periodically to remove accumulation of dirt.

Vehicle Use Policy Polaris Ranger Polaris is to be kept in the trailer at all times when not in use. Keys are not to be left in vehicle, inside trailer or inside towing vehicle. Doors are to remain on vehicle at all times. Windshield and rear glass are to remain on vehicle at all times and are to remain closed when Polaris is not in use.

Vehicle Use Policy Polaris Ranger Polaris is to be positioned in trailer to provide a level loading across axles. Vehicle is not to be used when usage hours indicate service is needed. Service is required after the initial 25 hours of operation and every 50 hours afterward. Engine oil and fuel are to be check prior to each use of vehicle. Winch is not to be used to pull stuck vehicles other than Polaris. Winch may be used to aid in removing boats from ponds if used properly.

Vehicle Use Policy Polaris Ranger Polaris is to be kept clean inside and out. Debris and dirt are to be swept from cab after each use and vehicle should be washed if excessive dirt accumulates on outside of vehicle. NOTE: Do not use high-pressure wash equipment as this could damage vehicle – use hand-held hoses only. Do not wash inside of cab. Inside of cab shall be wiped down with damp cloth to remove dust. Miscellaneous equipment is not to be stored permanently in vehicle unless the equipment is part of the original equipment provided with the Polaris. Vehicle shall not be used in any fashion for which it is not intended. (reference standard operating procedures for proper use) Vehicle is not to be operated at excessive speeds.

Vehicle Use Policy Polaris Ranger Tools and equipment are to be carried in the bed of the Polaris, with the exception of GPS equipment which may be carried inside cap. Equipment should be secured to prevent movement and possible damage to Polaris. No food or drinks are allowed in cab. No smoking or tobacco products are allowed in cab.

Standard Operating Procedures Effective Date April 2006 General Operating and Inspection Procedures Start Polaris and back from trailer – use spotter to insure that door cables are not damaged during backing procedures. Place foot on brake prior to starting. Do not release parking brake until ready to back. Do not use excessive speed. Polaris is susceptible to roll-over when used at high speeds in reverse. To start cold engine, pull choke all the way out, do not depress gas pedal or crank for more than 5 seconds. After starting, push choke in half way. Once engine is idling smoothly, push choke all the way in. Choke is not required for warm engines.

Stop Engine, set brake Completely fill out walk-around inspection form Load equipment in back of Polaris and secure. Do not exceed maximum load for vehicle of 1750 pounds (including operator and passengers) or 1250 pounds in the cargo area Fasten safety belts and restart engine. Place foot on Brake prior to shifting gears. Shift vehicle into appropriate gear, release parking brake and release active brakes.

General Operating and Inspection Procedures Do not shift gears with vehicle in motion. Do not switch between all-wheel drive and rear wheel drive while in motion. Severe damage will occur to transmission and differential if gears are shifted while in motion. Polaris should be operated normally in low gear while surveying. High gear may be used while traveling to work site only when on hard surfaces and lightly loaded. High gear should not be used when towing objects or carrying heavy loads. equivalent to initial oil change time of 25 operating hours. During break-in period, do not operate vehicle at maximum RPM or at steady RPM for extended times. Do not allow vehicle to idle for extended periods of time. Break in period is Proceed to work area. Do not drive on roads unless surveying roads. Maintain safe speeds.

Standard Operating Procedures Effective Date April 2006 General Operating and Inspection Procedures Do not operate vehicle on slopes in excess of 15 degrees or in areas with excessive loose materials. Do not drive through standing water unless through a designed low-water crossing. At no time should equipment be driven through water that exceeds the depth of the floor boards. If vehicle has been operated in water, notify Owner supervision. Specific maintenance items must be completed before next use of vehicle to prevent damage. When climbing or descending hills, do not traverse at an angle. Go straight up or straight down. Maintain steady speed that is not excessive. Do not make sharp turns

Standard Operating Procedures Effective Date April 2006 General Operating and Inspection Procedures Use chocks and parking brake when parking vehicle at work site Do not leave Engine running while unattended No persons are to ride in rear of vehicle Seat belts are to be used at all times No more than three persons may ride inside vehicle

General Operating and Inspection Procedures Windows are to remain on vehicle at all times Side Curtain window and front windshield may be opened to provide ventilation during hot conditions Return vehicle to trailer when job is complete Remove all equipment from vehicle other than those that are part of vehicle. Secure vehicle in trailer using haulage straps. Do not attach straps to axles or control arms for wheels. Utilize safety chain attachments on rear of vehicle and holes in skid plat in the front.

Towing Proceedures Do not tow standard sized vehicles with Polaris Light equipment such as boat trailers can be towed using equipment Use proper receiver hitch and ball size for equipment being towed Do not exceed the towing capacity of Polaris Insure that safety chains (if equipped) are attached and that ball hitch is secured. Do not turn sharply while towing equipment. Equipment may contact rear of vehicle or tires and cause severe damage

Towing Proceedures Utilize slow speeds when towing equipment Polaris is not to be used to pull any stuck vehicles out Do not exceed maximum towing capacity on level ground of 1750 pounds or 1000 pounds on slopes. Do not exceed hitch weight of 150 pounds.

Use of Electric Winch

Standard Operating Procedures Effective Date April 2006 Use of Electric Winch Electric winch is to be operated using the controller provided by the manufacturer Wear heavy leather gloves when handling winch cable Winch should be operated in such a manner as to avoid the direct line of pull on the cable. Operator should stand to the side of the vehicle while operating Capacity of winch should not be exceeded Winch should not be used to pull vehicles larger than Polaris

Standard Operating Procedures Effective Date April 2006 Use of Electric Winch Winch cable should be inspected for damage prior to loading winch Winch should not be pulled tight against guides. Brakes on Polaris must be set if pulling on other objects. Vehicle should be in gear and pulling (operator in cab) to assist winch if winch is used to extricate Polaris (requires two operators) Never shock load winch or winch cable.

Use of Electric Winch Do not do continuous side pulls as this will pile cable, resulting in damage to cable and winch. Do not overheat winch motor

Standard Operating Procedures Effective Date April 2006 Removing Stuck Vehicle Should the Polaris become stuck, electric winch should be used to assist with extricating vehicle if possible Do not pull on bumpers of Polaris with a second vehicle If winch cannot be used, tow straps may be attached to frame of Polaris to attach to other vehicles If possible, obtain help and push vehicle out of mud by hand

Connect Vehicle to TrailerBlind Spots, Pinch Points, Equipment Utilizing help, back vehicle into proper position to connect trailer. Spotter should be located out of line of fire to connect trailer. Spotter should direct backing operations and connect trailer to towing vehicle. Trailer should not be connected until towing vehicle is in park and the parking brake is set. Operator of towing vehicle must remain at driver controls. Connect electric connector to vehicle to enable lights and brakes. Verify operation of all lights and note on pre-op. Raise jack stands at rear and front of trailer completely Spotter and Operator must maintain clear communication, and the spotter must not be located between the trailer and tow vehicle during any movement.

Avoid abrupt or sharp turns, stay on level roadways. Increase following distance to account for increased stopping distance resulting from trailer and equipment. Never leave roadways.

Disconnect electrical connection and secure front of trailer with lock when clear

Close and secure both doors on trailer.

Start engine and travel to work area. Do not drive on roadways unless required to access work area. Maintain proper speeds to maintain control. If entering active mine areas, notify loading and hauling crews prior to entering area. Do not traverse any slopes greater than 15 degrees. Travel up and down slopes instead of cross slope if possible. Utilize proper gearing while traveling. Reference SOP for proper gearing and use.

Avoid any areas with water unless area is marked as low-water crossing or water is confirmed to be lower than allowed for travel of Polaris according to SOP and operators manual. Maintain proper speeds. Do not make abrupt turns. Travel up and down slopes if possible. Never traverse a slope in excess of 15 degrees.

Open trailer side door and rear ramp. Insure that jack stands are deployed and trailer is stable and secure. Using lowest gear and minimal speed, drive Polaris into trailer, centering on ramp. Pull forward to center weight of Polaris over axles of trailer. Put Polaris in neutral, set parking brake and kill engine. Secure Polaris to trailer using tie-downs and travel straps. Only connect travel straps in locations used during unloading. Never connect tie down straps to brush guard or body of Polaris.
