Welcome to my meeting. Today we are going to plan my program for next school year.

My name is James Tully I am a student at Hutchison High School. I am 15 years old. Will everyone please introduce themselves and sign into the meeting.

Things that have been easy for me last year are: Health went well, taking notes and taking tests (multiple choice/TF, fill in the blank with word banks) Organizing electronic files, daily assignments

Things that have been hard for me last year have been: Geometry was difficult, theorems and proofs Homework in math (didn’t take advantage of after school assistance) Staying organized with daily planner

When I graduate from high school, I want to: Work: Game programmer Education: Binary/Hexadecimal (IT), program Live: Apartment/Dorm Community: Exploratory Daily Living: Independent Please review my Transition Plan

For me to be successful in school I need: I need to remember my planner, staying organized (binder) Study guides at least a week in advance
