In the Beginning…… World Café brain storming – what would the Integrated Health Network look like? A partnership with facilities outside of a hospital setting Community integration Opportunities for people with chronic conditions No cost for any of the programs/appointments

Who was at the World Cafe? Health care providers Dietitians Nurses Social Workers Community Care Occupational Therapists Community Recreation representation

The Process The Integrated Health Network staff consisted of: 2 nurses, 1 dietitian, 1 social worker & 1 receptionist (Medical Office Assistant) IHN Staff would consult with patient who was referred by their physician, meeting the criteria of 2 chronic conditions, offering them choices of: footcare, Mindfulness, The IHN staff would then refer the person to a Strathcona Gardens exercise program

The start of partnership Screening consultation to analyze level of physical ability Up to 24 clients 2 x per week 4 classes per week Aquatic program only $ seed funding 24 week program First “physician appointment” at a Recreation Centre with 15 patients -

2015 Screening consultation to analyze level of physical ability Up to 210 clients 2 x per week classes per week Classes fluctuate depending on client level ability - post assessment

Energy PLUS……

All IHN Staff and Clients Invited Twice a year a Pot Luck luncheon is offered to all of the clients Tables are set with real cloths and flowers The “RECKIE” side of this program is “enjoyment” A skit or song, is produced as entertainment

Education & Antics How to make the program successful: A combination of “smarts” and “Silliness” Well trained staff – Kinesiologists/Exercise Therapists, Personal Trainers, Older Adult certified, Aquatic Therapy & Fitness Certified Lifeguards, CSEP - CEP This program is serious FUN! Functional training for Sit to Stand – ½ kneel to stand at pool side MUST know how to work with people

Basic improvements in all testing

2016 and Beyond Continuing to analyze level of physical ability Over 1500 clients through the program Focus on “on/off floor transfers” Adding a specific Fall Prevention program in Water & on Land Postural adjustments The IHN is now evolving, however the process continues to develop……. with the same group of caring staff at the hospital and Regional District
