More focus on preventative health Improved access to mental health Better use of technology Greater use of medicines More care closer to home More joined up health and social care Improved access to primary care Focus on the elderly and ageing population Maintain choice and waiting times performance Improve community transport Self help and advice Working with our community to set priorities

Major achievements over last two years Engagement and involvement of 32 member practices, development of locality structure Financial stability, delivery of efficiencies 2011/12 and 2012/13 Developing clear strategies: - Medicines management - IT and Informatics - Quality Governance Framework Working closely with our two local providers - Key role in the Joint Services Review Service transformation: - Urgent care pathway - ICATS - End of life care CCG Authorisation process - No conditions, top 10-15% in country

Challenges for 2013/14 Managing the transition from NHS Worcestershire, working with the NHS Commissioning Board and local providers to be clear on roles Maintaining engagement with our member GP Practices and developing their understanding of the commissioning role Building relationships and our community engagement capacity Improving the quality of some key services – urgent care, stroke Working with partners to bring the Joint Services Review to a conclusion and developing our Integrated Care work Managing demand – three key areas identified: 1.Emergency admissions for children 2.Care of the frail and elderly 3.End of life care Key themes across all plans: Quality, safety and clinical effectiveness comes first Prevention is better than cure Care is coordinated across the health and social care economy Independence is promoted and health is made personal for each individual Maximum value is gained from every health pound spent.
