A childfocused healthcare – a challenge for Europe Björn Wettergren MD, PhD Member of the EC of the European Academy of Paediatrics (EAP)

Mortality 0-14 in European countries I. Wolfe et al

If the countries with the best results should be the norm there are about 6000 avoidable deaths per year among children 0-14 years in Europe Lancet, March 2013, Ingrid Wolfe et al

Important factors for progress High paediatric competence both in inpatient and outpatient care A preventive child- and youth health care that reaches all A preventive focus in all sectors of the society

Exposure for tobacco smoke at home at 8 months of age

Child injury mortality rate Death per children 0-14 years

Safety seats The first product on the market was developed in Sweden. At the start in the 1980s parents to newborns could borrow the seats from the Child Health Care Centres financed by the county. Accident prevention is included in the Child Health Care program and starts with a home visit when the child is newborn

Paediatric primary care in Europe A great diversity!

It depends on age…

How can we learn from each other? Do not focus on organizations, look at the processes Study the pathways Analyse the background when not doing so well See every missed child as a failure

How can we meet the needs of children at the community level? Keep the aim of highest possible paediatric quality close to the child’s environment There can be a value of merging to group practises at least in urban areas and work in teams with other professions Secure close cooperation with relevant sectors within the community, – Preschools, schools – Social welfare units – The care for handicapped children – Mental health care

How can we meet the needs of children at the community level? Secure close cooperation with the paediatric departments and the children’s hospitals. Use the possibilities of the internet and gather round the child in stead of referring the child in different directions.

EAP statement, Lancet May 2015 Children have the right to access the highest possible standards of health-care services and facilities, both in primary health care and when they need specialised care.

EAP statement, Lancet May well organized paediatric primary care system and special knowledgde, ethics and empathic behaviour of the paeditric healtcare provider is needed..any restriction of provision of appropriate care would contradict article 24 of the UN Convention on the Rights of the child

EAP statement, Lancet May 2015 A structured and accountable paediatric training programme is strongly advocated for all doctors providing first-line care to children in primary care and where and when possible, it is reccommended that paediatricians should be providers of paediatric primary care. When family doctors provide primary health care, a close collaboration is encouraged with paeditricians and adequate continued training in both paediatrics and priamry care. All children need timely access to paediatric advice, which should be available in the community and link to specialised care in hospitals.

You have a golden egg because the PCPs are already there!

Do not crack the egg by reorganisation, invest in it and develop it further…….

..and it will end up in a full blooded rooster…

…to guard and take care of…..all the needs of the child!
