Access and Transport Progress to Date: Next Steps: Patient Care Services – alignment between transport and clinic times NEPT S Business Case approval Analysis of current patient flows to Outpatients (OPD) and Day Surgery New OPD call centre in Hywel Dda providing opportunities for alignment (e.g. zoning of high volume patients / clinics) Implementation of NEPTS Business case Development of enhanced Country Cars model in partnership with Third Sector Meeting of sub group on 30 November Workshop in January 2016 to develop pilot for OPD alignment Emergency Services: Model live on 1 October Focused Action plan for Hywel Dda /Powys Deputy Head of Operations Daily monitoring Opportunities to expand co-responder (MWWFRS) Continued development of APPs Implement Alternative Transport Initiative Working with Primary care and Out of Hours services

Access and Transport Patient Transport/Non Emergency Patient Transport Service Matching of clinic times to transport availability and planning process Type of transport better matched to patients clinical/social needs Reduction in carbon foot print – less unnecessary travel e.g. Telemedicine Will improve wellbeing for staff which will ultimately lead to better service for patients Clinical Response Model Change is based on doing what is best clinically for all patients (not just Red calls) aim is to send the right resource (skills and vehicle) the first time. E.g. send an ambulance if conveyance is likely, rather than sending a Rapid Response Vehicle to ‘stop the clock’ and request for ambulance back up to convey.
