Interchange LANE Best Practice “Associations as LANE Convener"

"Associations as LANE Convener" Problem Statement: New York State had no LANE convener and no formal participation in Advancing Excellence (AE). Goal: Establish a New York LANE convener to support state nursing homes in quality improvement and participation in AE campaign. Solution: Four concerned provider associations joined forces.

Continuing Care Leadership Coalition (CCLC) Healthcare Association of New York State (HANYS) New York Association of Homes and Services for the Aging (NYAHSA) New York State Health Facilities Association (NYSHFA)

Issues Identified Diminished nursing home participation in AE quality improvement (QI) campaign –Providers questioning “another new QI program” and their investment –Providers concerned about confidentiality of their data reporting of AE goals Need for a recognized and trusted organization(s) to lead the initiative –Four associations familiar to nursing homes –Need for branding of a new QI resource and as state link to AE Need for easy, efficient communication across our large state –Provide information and education to nursing homes efficiently –Communication and information to LANE members Need for resources to support activity –Each convener partner abilities/limits –Recruiting LANE members

Timeline of Partnership Activity May 2007 – There 47 New York State Nursing Homes participating in the Advancing Excellence Campaign June 2007 – CCLC, HANYS, NYAHSA, NYSHFA joined together to form the Empire Quality Partnership Mid-June 2007 – Empire Quality Partnership began reaching out to NYS organizations and invited them to join as LANE partners August 2007 – The collaborative launched the official Empire Quality Partnership website and the server January 31, st Statewide Teleconference: Achieving Goals 1 April 22, 2008 – 2 nd Statewide Teleconference: Achieving Goals II: Consistent Assignment and Staff Retention Date September 26, 2008 – 3 rd Statewide Teleconference: Achieving Goals III: Resident Satisfaction. November 2008 – NYS Board of Examiners of Nursing Home Administrators agreed to sign on as NYS LANE participant December 2008 – There are 132 New York State Nursing Homes Participating in the Advancing Excellence Campaign
