Current projects PAH Front Door A&E – GP and Navigator (1 year) PAH Paeds Front Door – GP PM GP Access Fund (with Uttlesford Health) Epping and Harlow – Central Referral Service (CRS) (Orthopaedics, Ophthalmology, Minor Surgery, Dermatology, Gastro) DVT – from 1 April

PM GP Access Fund Formally PMCF Bid December 2014 Joint bid with Uttlesford Health (36 practices) £2.6m – 1 year pilot Started October End March 2017? Improve primary care access for WE patients Facilitate 7 day provision of ‘routine’ care Reduce A&E attendances Embrace technology Operations team – 5 FT and 3 PT staff

Phase 1 – Weekend appointments Aim 0800 – 2000 Sat & Sun Started 3 Oct – 20 patients – single hub (GP Sat, GP + HCA Sun) End Nov – 5 hubs (not all 8-8) Last weekend – 900 appts available 600 patients booked. Nearly 7000 so far (19 GP’s, 10 nurses, 5 HCA’s) IT solution – 2 clinical systems – seamless patient flow

Who are we seeing? Working age adults (20 – 65) – 70% Acute exacerbation of chronic issues Time off work! Asthma, diabetes, COPD Cervical screening, dressings

Patient feedback Very positive 97% satisfaction 98% would recommend service Smiley faces! Staff feedback – enjoy the environment – no phones!

How do ‘you’ get seen? Local GP surgery Reception team book appt GP, nurse or HCA SHC manage booking Referred in Discharged back to GP practice

Phase 2 – weekday appointments Weekday appointments Monday to Friday, 1830 – 2000 Neighbourhood based? 8 n’hoods in WE Face to face, phone, triage – all tbc Pilots about to start – March and April

Phase 2 – video consultations Pilot Limes – demonstrated technology Issues – platforms, time to set up call, IT skills More pilots with local practices Integrate with weekend and weekday services Disease specific pilots – eg diabetes in U25’s

Other projects Self care/signposting website/phone app Health Help Now – from July for 12 months Available to all WE patients Patients surveys – tablets and WiFi – no paper! Started last weekend

Other projects Mobile working - use of laptops and iPads by GP’s for home visits Care Homes – integrating technology into Care Homes Pilots about to start Technology project with St Clare Hospice Video conferencing for education
