MPHCA CHIP OUTREACH and ENROLLMENT GRANT Background and Overview May 24 th, 2010 Carley Jefcoat and Mitch Morris

Project Design and Goals  Overall goal: Enroll 24,000 children in MS CHIP/Medicaid health coverage during the 15-months of active outreach efforts.  Outreach and enrollment activities will be conducted by Community Outreach Specialists (COS).  COS staff will be stationed statewide in Mississippi’s 21 Community Health Centers (CHCs).  Collect project data and document impact

Roles and Responsibilities of Participating Agencies  Mississippi Primary Health Care Association (MPHCA)  Lead agency of project  Serve as liaison for MPHCA and DOM  Division of Medicaid (DOM)  Conduct eligibility training for COS staff  Facilitate project involvement with regional DOM staff  Provide statewide and regional project data  Community Health Centers  Employ and provide support for COS staff  Provide supervision of COS staff

MPHCA Outreach Coordinator  Monitor project from statewide perspective  Collate and analyze data  Support data collection software  Conduct site visits  Additional training as needed  State/Federal project reporting

COS Roles and Responsibilities  Provide enrollment assistance to potentially eligible families  Conduct community-based outreach and collaborate with other community-based organizations to promote enrollment assistance  Collect and report project related information

Provide Enrollment Assistance  Source of potential clients  Referrals from CHC  Referrals from external agencies  One-on-one assistance  Eligibility  Application completion  Supporting documentation  Facilitate Medicaid interview  Annual redetermination

Provide Enrollment Assistance  Information/Data Collection  Collaboration with local Medicaid Office/OEWs  Select best-fit interview site  Resolve application status  Productivity Targets  1000/FTE/Year  Approximately 4 completed applications per day

Outreach and Collaboration  Examples of outreach activities  Health fairs  School functions  Group presentations  Distribute flyers/brochures  Identifying potential partners and community contacts  Head Start Centers  Churches  Other health and social service agencies  Complete and submit monthly reporting form

Project Reporting 1. Enrollment data will be automatically captured through use of project software 2. Monthly outreach reports will be submitted to MPHCA

Time/Calendar Management  Overall goal: Enroll 24,000 children in MS CHIP/Medicaid health coverage during the 15-months of active outreach efforts.  Outreach activity is intended to support successful achievement of the project goal.  Outreach activity should not be conducted at the expense of enrollment.  Each COS should work with their supervisor to determine the best approach for managing these responsibilities.
