Investing in Communities Early Milestones in Aging and Public Health 2003 CDC and AoA released “The Aging States Project: Promoting Opportunities for Collaboration between the Public Health and Aging Networks” CDC and AoA, in partnership with NACDD and NASUA, initiated support to public health and aging services agencies in 10 states to conduct year-long projects promoting older adult health (Opportunity grants) 2005 AHRQ, AoA, CDC, and NIH sponsor “Evidence-based Disability and Disease Prevention for Elders: Translating Research into Community-based Programs”

States Receiving Healthy Aging Grants 2003–2010 (number indicates how many grants received over 7 years)

CDC Investing in Communities Pioneering Healthier Communities (Healthy Communities) Over 100 communities are provided seed funding for implementation of policy, systems, and environmental change ACHIEVE (Healthy Communities) Over 100 communities develop a community coalition, receive training and technical assistance, and are provided seed funding to implement policy, systems, and environmental change strategies Strategic Alliance for Health (Healthy Communities) 14 rural, urban, and tribal communities develop effective strategies to address chronic diseases and mentor other communities through the development of action guides

Collaborative Chronic Disease Prevention and Health Promotion Program DHHS announced the availability of $40 million to strengthen and better coordinate activities within state health departments aimed at preventing chronic diseases and promoting health CDC expects to award funds for 3-year coordinated statewide chronic disease programs to all 58 U.S. states and territories, with approximately $40 million available for the first 12-month budget period.
