4 MSHOP Compass Workers  Paid peer support for carers of people with moderate to severe dementia living at home  Bridge between carers and services, emotional support and navigation of “maze”  Flexibility: ○ “out of hours” service if needed support after move into care ○ support after bereavement

7 Adult Mental Health  A dedicated AMH lead  A Carers, Family and Friends Forum happens bi-monthly  All 40 of the teams have a designated Carer Lead  All teams have utilised the Triangle of Care Audit Document  Some teams also have a designated Family Work Lead  City Carer Support Service  Open Dialogue Training and Steering Group  Hosted a number of Conferences and Think Family roadshows via the Live Project.

9 “We have a monthly forum where past, present, future and local carers, family and friends can come along to meet other carers, whilst having support from the carer champion, carer leads and the carer peer support volunteer. We have guest speakers attend the forum and we also have evening meals to socialise which people really seem to enjoy. Recent and forthcoming speakers include Pharmacy, the consultant, peer support worker, solicitors to discuss Wills, Probate and Lasting Powers of Attorney”. 10 CAMHS Services  Self Harm Team ○Information evenings with guest speakers  Self-harm assessments at QMC ○Parents/carers are part of the assessment process and are seen separately and then are part of the management/care planning ○Receive copy of care plan

11 CAMHS Services  Adolescent Unit ○Weekly Step Ahead meetings following ward rounds to meet with parents and young people. ○Families are invited to attend CPA/CPA every 4-6 weeks
