 Subaward Request Form Sole Source Justification Multi-Campus (MCA) Commitment Form

 Subrecipient Commitment Form This form is completed by Subrecipient’s authorized official. It provides information about Subrecipient and status of its compliance and administrative readiness to perform its activities.  Subaward Verification Form This form is completed and signed by the PI/PD; it is used to request a new Subaward or modification of an existing Subaward.

 New Form Replacing Subaward Verification Form;  Once our Prime Award or any subsequent modifications is processed (i.e. award/modification synopsis is issued), PI/PD will use this form to request new Subawards (i.e. Subcontracts and Sub-grants) as well as modifications to existing Subawards;  Signed by the PI/PD;  As a part of this new procedure, departments/PIs/PDs don’t need to go through their C&G officers for subsequent modifications to Subawards that were approved by the sponsor;

 Completed & Signed Form should be sent to:  They would however need to work with their assigned C&G officer to request prior approval for any new subaward that was not included in the original approved proposal(if/when required per sponsor policy or specific award requirement);  We will continue to accept Subaward Verification Form during a phase out period ending on June 30,  We will not use/accept the Subaward Verification Form effective July 01,  Please note that SPA will not work on any Subaward related transaction (i.e. new, continuation, no cost extension, etc…), before receiving a completed and signed Subaward Request Form from department/PI/PD. It is the responsibility of department/PI/PD to use Subaward Request Form to initiate any and all Subaward transactions.

 You will need to complete this form, if your Subaward is under a Contract (Subcontract), or  You are requesting a Subaward under a Grant or Cooperative Agreement that was not part of the original sponsor approved proposal.  There is a link to this form in Subaward Request Form.

 MCA Commitment Form is a shorter version of our Subrecipient Commitment Form geared towards other UC campuses.  Sponsored Project Administration and/or its equivalent authority within Subrecipient UC campus needs to complete and sign this form.
