MACROSCOPIC ANALYSIS OF HUMAN MASSETER COMPARTMENTS ASSESSED BY MAGNETIC RESONANCE IMAGING Aims of the study: The human masseter is divided into compartments by aponeuroses. Nonethess, the qualitative and quantitative features of these compartments and their aponeuroses have been scarcely investigated. This study investigated the three-dimensional compartmentalization of the masseter muscle in vivo and tested the hypothesis that aponeurosis content varies systematically across different masseter subportions as well as between genders. Materials and Methods: 14 healthy participants, without signs or symptoms of temporomandibular dysfunction (7 M, 7 F, mean age ± SD= 26.0 ± 2.5 years) were recruited. Coronal Magnetic Resonance (MR) scans of the right masseter muscles were obtained by means of a 1.5-tesla scanner (Gyroscan Intera, Philips Medical Systems, Best, The Netherlands) using T1-weighted sequences. Slice thickness was set at 2 mm, and pixel spacing at 0.51 mm. Individual scans of the right masseter muscle were imported and analyzed by AMIRA (5.2 Visage Imaging software, Berlin, Germany). Muscle boundaries and aponeuroses were segmented manually (fig.1 ). A set of points and reference planes (fig. 2) was constructed in order to subdivide each masseter into eight subvolumes (regions of interest). The volume of masseter aponeuroses for each masseter subvolume was assessed via software. Conclusions: The internal architecture of the masseter muscle varied markedly across individuals. The aponeurosis content varied systematically across masseter subvolumes and did not differ between genders after adjusting for body height and weight. Aponeuroses contribute to the storage of elastic energy and play an important role in protecting muscle fibers from contraction-induced or high-strain injuries. Thus, the reduced amount and volume of masseter aponeuroses in females might represent a risk factor for postexercise masticatory muscle pain. - Augat P., F. Results: The mean ± SD volume of masseter muscle was 28.8 ± 7.7 cm 3 and was significantly influenced by gender (F = 22.6; p < 0.001), being lower in females (22.0 ± 5.9 cm 3 ) than in males (33.4 ± 3.0 cm 3 ). This difference, however, was not statistically significant (F = 4.5; p = 0.06) after adjusting for body height and body weight. Aponeuroses were widely represented inside the masseter, amounting to 7.1 ± 2.1% of its volume. They varied significantly across the different masseter subvolumes (F = 21.9; p < 0.001), with the highest content in the most anterior and cranial subvolumes. There were more aponeuroses in the deep than in the superficial masseter muscle subvolumes. The mean ± SD volume of masseter aponeuroses was 2.0 ± 0.8 cm 3, and was significantly influenced by gender (F = 7.4; p = 0.018) amounting to 1.5 ± 0.6 cm 3 in females, and to 2.5 ± 0.7 cm 3 in males. However, the difference was not statistically significant after adjusting the model for the effect of body height and body weight (F = 3.0; p = 0.08).
