Action items: Module 1 It is crucial that you and your teams view the “Improvement science” webinar Run a query in your EHR to generate your practice or health center’s BP control rate and share it with your entire staff. Use the “Measure accurately” assessment tools to evaluate current BP measurement performance. Identify opportunities to improve patient positioning to ensure accurate measurement. Administer and debrief results from the “Staff hazard” assessment with all staff and prioritize two defects to reduce or eliminate. Discuss findings from the “Measure accurately” assessment tools on the next Share your experiences webinar. Create an action plan that will incorporate measuring accurately into the patient flow of your office.
