Pathophysiology  Functional (physiologic) changes in the body as a result from disease  Uses knowledge of basic anatomy and physiology  Includes aspects of pathology, which describes structural changes in body tissues caused by disease  Cause and effect relationships, defined by signs and symptoms, guide the study of a specific disease.

Definition: Hyperplasia  Increasing the size by increasing the number of cells  Physiologic  Hormonal: uterus, breast  Compensatory: liver  Pathologic: excessive hormone or growth factor stimulation

Definition: Metaplasia  Reversible change in which one adult cell type (epithelial or mesenchymal) is replaced by another adult cell type  Squamous metaplasia  Osseous metaplasia  Survival advantages but important protective mechanisms are lost  May predispose to malignant transformation  Reprogramming of stem cells to differentiate along a new pathway rather than a phenotypic change of already differentiated cells

Dysplasia  Literally means abnormal growth  Malignant transformation is a multistep process  In dysplasia some but not all of the features of malignancy are present, microscopically  Dysplasia may develop into malignancy  Uterine cervix  Colon polyps  Graded as low-grade or high-grade, often prompting different clinical decisions  Dysplasia may NOT develop into malignancy  HIGH grade dysplasia often classified with CIS

Liquefactive Necrosis  Digestion of tissue  Results from ischemic injury to the CNS  Focal bacterial or fungal infections  Microbes stimulate accumulation of inflammatory cells  Enzymes of leukocytes digest (liquefy) tissues

Liquefaction Necrosis in the Brain

Coagulative Necrosis  Underlying tissue architecture is preserved  Affected tissues  firm texture  Denatures structural proteins and enzymes  blocking the proteolysis process  Recruitment of leukocytes  digest dead cells by leukocytes lysosomal enzymes  Cellular debris removed by phagocytosis  Characteristic of infarcts in all organ EXCEPT the brain

Coagulative Necrosis of the Kidney

Fat Necrosis  Focal areas of fat destruction  Trauma to tissue with high fat content  Leakage of pancreatic lipase  digest tissues  fatty acids released from digestion form calcium salt  Digest blood vessel  hemorrhage

Fat Necrosis in the Mesentery

Caseous Necrosis  Has a cheese-like (caseous, white) appearance, gross examination  Histologically: granuloma  Example: tuberculosis

Necrosis  Infarction  Area of dead cells as a result of oxygen deprivation  Gangrene  Area of necrotic tissue that has been invaded by bacteria

Gangerenous Necrosis  Not a distinctive pattern of cell death but still commonly used in clinical practice.  Lost of blood supply to tissues  coagulative necrosis involving multiple tissue layers.  When bacterial infection is superimposed, coagulative necrosis is modified by the liquefactive action of the bacteria and the attracted leukocytes (resulting in so-called wet gangrene)  Example: limbs
