Registration  All students MUST register to ACC PT is INVALID if not registered  Course Registration  Web-based Pre-registration in Sem 1, 2014/15  Add or Drop period in sem 2, 2014/15  Firms registration  Please inform DOA your PT firm 4

1. STUDENT ASSESSMENT FORM  Assessment by the firm supervisor:  Competency, Interactions, Work attitude and Self Development  Give to firm supervisor on the 1st day of report duty  Your responsibility to REMIND the firm towards the end of PT  The form is CONFIDENTIAL once filled by the firm supervisor.  to mail the form directly to DOA at the end of PT period.  to hand in the form to you in a SEALED ENVELOPE. 6

LOG-BOOK 7 DateNature of work doneSupervisor’s comment (weekly) Lecturer’ s remark 12/1/2014Observed stock count: - Audit objectives: Ownership – ensure no third party stock included in counting - Existence / Accuracy – checked the quantity in store with stock card.  A Diary – to record of duties/jobs  Date of Submission : Friday, 31 st July 2015 (before 4.00 p.m.).

3. PRACTICAL TRAINING REPORT  Summary of experience gained in PT  critical observations of the nature of work done  recommendations to DOA  Obtain the Guidelines on the PT Report from the DOA  Observe your grammar - generally should use past tense when describing your experience!  Date of Submission: Friday, 31 st July 2015 (before 4.00 p.m.). 8

4. INTERVIEW – LECTURER VS STUDENT  Each student is assigned one lecturer as DOA supervisor  List of Dept supervisor will be notified  Take note of the supervisor’s name, address. & contact no.  Please your supervisor during the first week : tell him/her that you have started your PT and your most recent contact no)  DOA’s supervisor will visit you at least once during the practical training period  Communicate through phone/ 9

5. INTERVIEW – DOA’s SUPERVISOR (LECTURER) VS FIRM’S SUPERVISOR  Each student is assigned one immediate supervisor at the firm  Should maintain good relationship with firm’s supervisor and consistently communicate with each other.  DOA’s supervisor will interview the firm’s supervisor – discuss about the student(s)’s performance 10

COLLECTIBLES 4 Documents that MUST be collected BEFORE you report for duty at the firm 1. Copy of DOA’s letter to the firm 2. PT Guidelines for students 3. Assessment form to be given to firm supervisor 4. Logbook All documents will be ready for collection at DOA 19 th Dec to 24 th Dec

SOME USEFUL TIPS – DOs  Attendance and Punctuality  Show high commitment and willingness to learn to assigned jobs  motive is to work & learn  Be proactive  Observe Islamic characters/behaviours  Professionally and Islamically dressed  Follow ALL DOA’s rules as well as the firm’s 12

SOME USEFUL TIPS – Don’ts  Avoid taking leaves such as going for vacation, getting married, or attending your siblings graduation ceremony  Medical leave - apply at firm and must inform Dept supervisor ( or phone)  Any leave more than a week, must be replace, i.e. extend PT  You are expected to complete 26 weeks  Avoid Office politics  Avoid problems with the staff/colleague For Preparation Practical Training report or daily log-book:  Avoid Last minute preparation  Avoid Plagiarism 13

Other Matters  Request to change placement  NO - We have already sent our confirmation letter to firms  EXCEPTION - e.g. you were bullied, sexual harassment, not allowed to perform prayer – PLEASE WRITE formal complaint to your DOA supervisor  After investigation, we may relocate the PT to other company  Formal Complaint or Request Must be in writing  Who to contact if I face problems related to PT? 1 ST – DOA supervisor (Assigned Lecturer) 3 rd – Student’s advisor 2 nd – PT coordinator 4 th – Head of Department 14
