CHALLENGES 2009/10 Continue move to Practice based care(65% plus) Look to expand Practice based teams/Diabetes education Skills /knowledge to convert Type 2 diabetics to insulin within primary care

CHALLENGES 2009/10 NEW Q.O.F TARGETS !!!!! HbA1C 7 or under > 50% HbA1C 8 or under > 70% HbA1C 9 or under > 90% Reduction in morbidity / mortality from diabetes OR Risks of over-medicalisation, iatrogenic illness and demotivated staff
