Building Aging at Home Invest $21 M in new funding over next 3 years … establishing the local Directional Plan Nine Focus Groups (talking to diverse groups of seniors) Seven Public Forums (one in French) Aging At Home Symposium (Paul Williams, Mark Nowaczynski - innovation and integration) … and the local Detailed Plan Three Public Validation Forums Over 1000 people engaged Services for Seniors Core Action Group

Year 2 – Learnings, so far Lots of diversion and prevention initiatives Need for unbundling Need to finalize costs and service levels Difficult to fund new start, high-cost proposals, notably supportive housing LHIN needs to work with group of providers to develop “direct / immediate” initiatives Always keep an eye on Integrated Health Services Plan and Health System Plan It’s about seniors and their health!
