Risk Assessment Process Bradford OHS Forum May 2016
Contents Approach Principles of Prevention Example Scenarios Summary
Approach When conducting Risk Assessments, treat this as an OPPORTUNITY TO IMPROVE and make things safer. Risk Assessments are not: A paperwork (tick box) exercise A reason to prevent work activity happening The “Be All and End All”
Approach Risk Assessments provide an opportunity to: Robustly examine work activities within an organisation Engage with the people conducting the activity Identify significant hazards associated with the work Improve job or equipment design to eliminate risk Review existing control measures Review training and competence requirements IMPROVE THE WAY THINGS ARE DONE!!
Approach Plan Manage Monitor
Principles of Prevention Eliminate Hazards at source Reduce Risk to lowest level possible Inform Provide information relating to residual risk Control Suitable measures to control residual risk
Eliminate Do we actually need to carry out the work? Job Design? Equipment?
Reduce Alternatives? Equipment Substances Process Control Measures? Reasonable Consistent Reliable
Inform Signage Operation & Maintenance Instruction Training Staff Engagement Consultation Written/Verbal
Control Management and Monitoring Safe Systems of Work PPE
Process Identify Hazards Identify who is at risk Is the risk acceptable What do we need to do to control it? What information and Training is required? Review Effectiveness
Risk Rating 4 key questions to ask: 1.What is the potential for harm (if the hazard were to result in injury or damage)? 2.How likely is this harm to occur if the hazard were not controlled? 3.Are the existing controls sufficient? 4.If not, what do we need to do to reduce risk to the lowest practicable level?
Risk Rating Severity FatalityMajor Injury Reportable Injury Minor Injury Likelihood Highly Likely Likely Unlikely Highly Unlikely
Scenario 1 – Electrical Work
Scenario 1 What are the Hazards? What is the potential for harm? How likely is it to occur if uncontrolled? Control Measures?
Scenario 1 Hazards Electric Shock Fire Potential for Harm Fatality Major Injury Likelihood Likely (?) Control Measures Training Inspection Signage PTW Competence
Scenario 2 – Work at Height
Scenario 2 What are the Hazards? What is the potential for harm? How likely is it to occur if uncontrolled? Control Measures?
Scenario 2 Hazards Falls from Height Falling Objects Potential for Harm Fatality Major Injury Likelihood Likely (?) Control Measures Job Design Equipment Training PTW PPE SSW Signage
Summary Be realistic when assessing risk (don’t over prescribe) Engage the people doing the work Adopt ERIC Adjust job design and equipment where possible Communicate your findings Identify any necessary training requirements Monitor and Review
