Students will be able to make a medical appointment in a simulated telephone call and write down their appointment time correctly. Students will use the calendar and appointment form at the end of this assessment to write their appointment time. Teachers will staple the calendar / appointment form to the assessment.

Zero (0) points Content B Points Appointment date and appointment time are written correctly. Two (2) points Only one (date or time) is written correctly, the other is missing or incorrect. Task 2: “Complete a Medical History Form” All Levels Given an agency-created patient medical history, Intermediate Low - Advanced students will complete an authentic medical history form. Beginning Low and Beginning High students will complete an authentic medical history form with: aREDUCEDnumber of sections.

Task 2: “Complete a Medical History Form” All Levels by reading a scenario of a patient’s medical history.Beginning Low: Directions: Read the following medical history of the patient and fill out the San Ysidro Health Center, Inc. Health History. Roberto Jajo is at the health clinic today. He is feeling very sick. His address is 2487 Main Street, EL Cajon, CA His telephone number is (619) He is married. He was born on February 3, His main problem is a fever and problems with his bladder. He did not have an electrocardiogram. His last tuberculosis test was normal. His urine test is not normal. He is taking one (1) medication. He takes Avandia. He takes two pills daily. He is allergic to penicillin. He had a previous urinary tract infection. He was in the hospital in March of last year. He does not use alcohol or tobacco.

Content A Task 3: “Visit a Doctor” During a simulated visit to the doctor, student will make up to 6 appropriate utterances, such as a greeting, a closing, and up to four (4) interactions with the doctor that include the description of the illness, symptoms, and responses to the doctor’s questions. Beginning Lowwill have two(2) interactionsfor a total of4utterances. Beginning Highwill have three(3) interactionsfor a total of5utterances. Intermediate and Advanced will have four(4) interactionsfor a total of6utterances. Content A Task 3: “Visit a Doctor” During a simulated visit to the doctor, student will make up to 6 appropriate utterances, such as a greeting, a closing, and up to four (4) interactions with the doctor that include the description of the illness, symptoms, and responses to the doctor’s questions. How does the student know what illness they have? The teacher gives an ailment card to a student. The student reads the information from the card they are given.

Content A Task 3: “Visit a Doctor” During a simulated visit to the doctor, student will make up to 6 appropriate utterances, such as a greeting, a closing, and up to four (4) interactions with the doctor that include the description of the illness, symptoms, and responses to the doctor’s questions. How does the teacher know what to say to the student (patient)? The teacher reads the information from the Doctor Treatment List(cards).

Content B Task 3: “Visit a Doctor” Student will use clarification techniques as needed in order to comprehend the doctor’s(teacher’s) diagnosis and instructions and take notes on them or remember them. Beginning Low Students: can have support such as a list on which they circle the information.

Content B Task 3: “Visit a Doctor” After the role play: The teacher: will check the student’s comprehension of the information from the doctor by asking up to two(2) comprehension questions and write the student’s answers on the assessment. If the student: takes notes on scratch paper to remember what the doctor said, the teacher will attach the note paper to the assessment. Beginning Low Students answer only one(1) oral comprehension question. Beginning High - Advanced answer two(2) oral comprehension questions. Beginning Low Students may use a list with circled information to answer.
