THE KENSINGTON The Kensington Opened September 2011 Assisted living (AL): 53 suites Memory care (MC): 34 suites in two neighborhoods NY licensure: “Enhanced AL” Licensed Capacity: 105 Urban location in “Medical Hub” Positioning – high-acuity, high-lifestyle AL & MC

Fragmented Care Loss of mobility reduces ease and frequency of PCP visits Lack of knowledge about services than can be provided in AL ER experience less than ideal Coordination of services post hospitalization Medical records are poorly communicated between community, PCP, and hospital COMMON PROBLEMS TODAY IN ASSISTED LIVING Implications: Lower quality-of life More acute episodes More frequent transfers Higher cost-of-care Shorter length of stay
