Site Selection Criteria – Health Centers Criteria and Scoring (1-5) Site Size  Site meets Banner's defined site area requirements  Site accommodates Banner Health Center template designs for building and site configuration Location  Proximity of site to population served in target area  Proximity to Banner metro area network (MAN)  Site has proximity to major roadway network  Avoid sites in or adjacent to residential areas (Neighborhood Model)  Avoid sites in or adjacent to residential areas (Community or Regional Model) Accessibility and Adjacencies  Site is accessible from a major thoroughfare (feeder for Neighborhood Model or Arterial for Community/Regional Model)  Site has the ability to incorporate median breaks or lights where required  Adjacent areas include complementary uses currently - retail/professional preferred  Adjacent areas can reasonably be anticipated to include complementary uses in the future  No major transmission lines are adjacent to the site
