Tower Hamlets Networks IC OneOld AUA DESIt looks like the AUA DES is to continue from Apr 16 CCG will have autonomy to incorporate with the Integrated Care Strategy IC TwoOld Int Care NIS Complex - Palliative, Nursing Home, Dementia, plus a discretionary group broadly (each Practice to be given an allocation calculated using 10% of your over 65s) IC Three Old Dia, COPD, Secondary Long Term Condition group Include ALL the patients in the NIS Care packages (DM, CVD = CHD, Stroke, PAD, AF, BP, COPD, Cancer care review newly diagnosed)

Tower Hamlets Networks MDTs IC OneN/A IC Two  Practice Int Care MDTs – Normally monthly but a min of 10  Network Education & Training on IC – awaiting Clarification Money the same as 15/16 IC Three12 MDTs – but any topics

Tower Hamlets Networks Finance Headlines  Monies the Same  Moving IC NIS (IC Two) to a 65% : 35% Performance payment  To date the IC NIS payment has been practice ‘activity’ based for 15/16 - now moving to Network Targets  Some items will be on performance eg. PAM – Last Years of Life review
