Our Successes CRCS EMR reminder has helped: - prompt caregivers to offer screening - clean-up data Reminder posters in exam rooms Purposeful cross booking with residents to discuss screening with patient Stop signs on EMR to highlight missing prevention data Verbal prompts by clinic aides to do screening Proper billing to alert Cancer Care Ontario to send a recall letter in two years

Sustainability with frequently changing interprofessional team CRCS Keeping it on the forefront: - monthly team meetings to report progress - orientation of learners Need for a full-time dedicated clinician to oversee the prevention process Need adequate admin staff for sustainability Ensuring data is clean by each individual team IT to link colonoscopies with FOBT data
