Kingston Hospital Previous audit showed poor cognitive assessment on admission to A&E (21%) Patients with cognitive impairment missed Missed opportunity for: – Specialist in-patient management – Appropriate follow up and referral to community support services

Allocation of junior doctor dementia champions for each ward Audit performed monthly for National Dementia CQUIN: – Screening question – AMTS within 72 hours – Appropriate investigations performed if AMT less than 8 – Appropriate community referral Wards bench-marked against each other and informed of monthly progress

Continuing the good work February 2013 audit results showed: – 91% use of screening question – 100% AMTS usage on admission – 100% referral to specialist services on discharge.
