AIM To reduce injury from falls by 40% from baseline by March, 2011. Increase the amount of clients identified “at risk” for falls to have a documented falls prevention intervention strategy (100%) by March 2011. Increase the amount of clients screened using a “falls risk assessment form” upon admission (100%) by March, 2011.
