Winter Comes but once a year………….. Working Together to Ensure we Keep the Health and Social Care System Moving Over Winter

What do we know From a social care perspective we start to see a spike in demand resulting from “winter” from mid November. –Respiratory –Frailty & Dementia –Falls –UTI People present at the hospitals from a variety of settings –Home with care –Home self care or Carers support –Residential/ Nursing Care Homes

What happens People are treated and their crisis resolved in the hospitals Evidence suggests that at the point their health crisis is resolved the best outcomes for people are achieved by them returning home. We know that Older People loose mobility the longer they stay in a hospital setting, some of which is never regained.

How can we Work Better Together? Home First What needs to be in place? What support is needed? How quickly can it be put in place? How do we manage expectations to make the outcomes positive?
