Background Developed an NHS Health Check paper for the Clinical Executive Committee in 2009 CEC identified early implementer practices Developed a service specification and standard operating procedures NHS Health Checks commenced with early implementing practices in 2009 NHS Health Check PMS Contract approved in 2010 Roll out across GP practices in 2011/12 Moved into LA building 1 st July 2012 New appointed Director of Public Health
