CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Governing Regulation CAP Regulation 62-2, Mishap Reporting & Investigation.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Definition CAPR 62-2, Mishap Reporting & Investigation, defines a mishap as "any unplanned or unsought safety event, or series of events, that result in or has the potential to cause death, injury, or damage to equipment or property.“ (The term “mishap” includes all accidents and incidents.)

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting The regulation requires wings to develop mishap-reporting procedures, published in a letter or supplement to the regulation. This letter or supplement must ensure that: u The wing commander and safety officer are promptly notified of all mishaps within the wing. u The CAP-USAF State Director is promptly notified of all mishaps within the wing.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting To satisfy this requirement, Colorado Wing has published Operating Instruction (OI) 11-24, Mishap Reporting Procedures. This OI may be found on the Colorado Wing CAP Website on the Policy Notices page. An Operating Instruction is directive. Compliance by COWG members is mandatory.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting According to this OI, Squadron and Group Commanders are responsible for ensuring that all mishaps involving members of their command or assigned equipment are immediately reported to the Wing Commander and the Wing Safety Officer prior to the submission of a CAP Form 78 Mishap Report.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting This notification to the Wing Commander and Wing Safety officer should be by telephone, using the numbers published on the Safety page of the COWG web site. The Safety page can be accessed by any member logged into the COWG web site. Click on COWG Services in the Site navigation pane and then click on Safety.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting This notification to the Wing Commander and Wing Safety officer is not complete until the notification has been acknowledged by the Wing Commander and Wing Safety Officer. A message left on an answering machine or voice mail does not satisfy the notification requirement. If you do not get an answer when telephoning, keep trying until you make contact.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting All Mishap Reports are filed using the on-line (Form 78) Mishap Reporting System in CAP eServices. While some mishaps may not qualify for statistical reporting purposes, there may be lessons to be learned from them or they may help in identifying safety mishap trends. Additionally, some mishaps appear to be minor in nature at first, but that may change with time. For that reason, all mishaps (accidents, incidents and minor mishaps) must be reported using the on-line reporting system.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting Unit Commanders/Activity Directors are responsible for ensuring an on-line Form 78 is accomplished within 48 hours of a mishap. There are no exceptions to this requirement! The Form 78 Mishap Report must be filed within 48 hours of the mishap! Do not delay the submission of the Form 78 Mishap Report beyond the 48 hour deadline while attempting to contact the Wing Commander and Wing Safety Officer. Make every possible attempt at contact, but do not exceed 48 hours before filing the Form 78 Mishap Report.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting The on-line Form 78 documents all mishaps and is an important legal document that must be completed correctly. Failure to complete an on-line Form 78 could result in the member being held personally responsible for damages or medical expenses incurred, and loss of government or corporate provided insurance benefits.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting Each unit is required to develop their own local Mishap Reporting Procedures. This requirement is found in CAPR 62-1, CAP Safety Responsibilities & Procedures, paragraph 3.e.(13) The Unit Commander, with assistance from the Unit Safety Officer, should ensure that this local procedure is available to all members and the procedure should be periodically briefed during safety education discussions.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Mishap Reporting u Unit Commanders must ensure that the “ball does not get dropped” following any mishap. u The appropriate notifications must be made to the Wing Commander and Wing Safety Officer prior to submitting the on-line Form 78 Mishap Report. u The on-line Form 78 Mishap Report must be filed within 48 hours of the mishap – no exceptions! u If unable to contact the Wing Commander and Wing Safety Officer, despite your best efforts, file the on-line Form 78 Report before the 48 hour deadline.

CITIZENS SERVING COMMUNITIES CAP Mishap Reporting Summary u Report all mishaps u Members follow local mishap reporting procedures u Unit Commander ensures notifications are made, by phone, to Wing/CC and Wing/SE before Form 78 submission u Unit Commander ensures on-line Form 78 is submitted within the 48-hour deadline. No exceptions!
