Pilot sites In patient medical ward – Thames District nursing services –Te Kuiti and Otorohanga In patient ward – Te Kuiti Hospital

What went well Engagement Patient centred conversations Initial feedback – Raises awareness of patient condition – Improves continuity – Easy to use – Senior nursing team very supportive of concept

Challenges VTE component – Confusing to staff – definition – Unsure of relevance to DN caseload – Awareness of poor compliance with VTE risk assessment at WDHB – should we use this measure or implement some improvement work around this first? Remoteness of services involved
