Older patients: difficult for them to remain living independently in their own home necessary for them to go into a nursing home where their symptoms and vital signs can be checked frequently.

Home Telehealth can connect a patient to a hospital from home using regular telephone lines Home Telehealth can help patients to remain at home and live independently.

Telehealth Advancement Act of 2011 in Section of the California Business and Professions Code Telehealth The mode of delivering health care services and public health via information and communication technologies to facilitate the diagnosis, consultation, treatment, education, care management, and self-management of a patient’s health care while the patient is at the originating site and the health care provider is at a distant site.

Telehealth facilitates patient self-management and caregiver support for patients.

Home Telehealth service that gives the clinician the ability to monitor and measure patient health data and information over geographical, social, and cultural distances.

This monitoring includes use of both video and non-video technologies.

Application of these technologies allows for: *increased access to health services *improved disease management *improved self-care management and earlier *proactive interventions for positive outcomes

How Telehealth Works Helping to improve clinical outcomes and reduced acute care hospitalizations Patient at Home Takes vital signs and customized surveys with monitoring devices.

Secure Data Infrastructure With specialized algorithms, data server processes clinical data into manageable information with bi-directional data communication with electronic health records (EHR).

4. Results are automatically sent to a website via a landline or cellular signal. Home telehealth Flow

5. The agency's telemonitoring nurse monitors the patient daily and alerts the patient and care team when results fall outside pre-defined limits. Home telehealth Flow
