Unit 8 Seminar Cathy Moore

This project is a little different:  You will use a hypothetical situation (again, follow your career path) to create a progress note.  Imagine that you are the professional meeting with the client. You will note the client’s progress towards goal completion while providing suggestions for homework or tasks for the client to finish before the next meeting.  You may use one of the past scenarios or you may create a new one.

 Write a progress note that summarizes the main points of the session, provides reflection as to how the client is meeting his or her goals, and includes a homework assignment. Remember that homework assignments can be as simple as asking the client to call a doctor to schedule an appointment or as complex as writing down actions in a behavioral journal.

Add specific notes that demonstrate your understanding of the importance of providing cultural-, gender-, or diversity- appropriate strategies to working with your target population. For example, in some cultures, it is disrespectful for children to make eye contact with adults and yet, lack of eye contact is sometimes noted as a problem in children by teachers. You might explain to the child that there are cultural differences between expectations of his parents and his teachers. Or, if you are working with a single mother, provide strategies for her to cope with her lifestyle rather than ignore the added stressors that face single parents. Be sure to include these types of descriptions in your progress notes to demonstrate your understanding of delivering client-centered care.

 Use the template provided for this project. (download the attached form and use it)  You are not writing a paper; however, you are still required to add between 800 and 1000 words to respond to the sections in the note.  Write using guidelines of Standard American English, and consult the Kaplan Writing Center if you have questions about grammar or format.  Remember that all work must be your own and that plagiarism is not tolerated. Be sure to review the plagiarism policy in your syllabus.

Progress Note Client name:Session Date/Time: Summary of Session: What was discussed? General “status” of client? Add specific notes that demonstrate your understanding of the importance of providing cultural, gender, or diversity-appropriate strategies to working with your target population. Change in Needs: Have client needs changed? New needs? Different needs? Resolved needs? Remember to prioritize needs – use sound reasoning in prioritizing needs Progress towards Treatment Goals: Think about what goals you might have set for this client (using what we learn tonight about goal setting) Provide reflection as to how the client is meeting his or her goals Homework: Include a homework assignment. Remember that homework assignments can be as simple as asking the client to call a doctor to schedule an appointment or as complex as writing down actions in a behavioral journal.

 Goals within counseling help to set the tone and direction one travel’s with their client.  Without goals, the sessions will wander aimlessly.  Both the client and the counselor should play an active role in setting goals

The client articulates where they want their counseling journey to take them.  Client role as one of driving the bus  Enhances sense of ownership and motivation – factors important in the change process Well identified goals help create a roadmap and means to evaluate Goals may change, evolve as therapy progresses

▪ Developmental- Assists clients in advancing social, cognitive, emotional goals. ▪ Preventive- helps client avoid undeserved outcome ▪ Enhancement goals -Recognition of the client's Special skills and abilities ▪ Remedial Goals- Assists clients in overcoming an undesirable development. ▪ Exploratory Goals- Examines options with client, trying new activities, relationships, etc.

 Goals are mutually agreed on by the client and counselor.  Goals are specific.  Goals are relevant to behavior.  Goals are achievement & success oriented.  Goals are quantifiable & measurable.  Goals are behavioral & observable.  Goals are understandable & can be re-stated clearly.

 Assist the reader  Be accurate and objective  Simple and direct style  Use basic English  Describe what happened  Avoid uncommon abbreviations  Record in a timely manner  Relevant significant information only

 Intimate or personal details that have no bearing on the client’s needs or situation  Gossip  Venting of own frustration or other feelings  Incriminating information to you or the agency  Inappropriate entries such as your own personal view of religion or politics etc.
