In-Network Information What does In-Network mean? 1) Using a doctor and hospital in Arizona 2) Blue Cross Blue Shield of Arizona providers. Employee Only – Deductible = $500 – Out of Pocket = $4000 Family – Deductible = $1000 – Out of Pocket = $8000

Out of Network Information What does Out of Network mean? 1) Using a doctor out of state 2) Not Blue Cross Blue Shield of Arizona Providers Employee Only – Deductible = $1500 – Out of Pocket = Unlimited Family – Deductible = $3000 – Out of Pocket = Unlimited

Win/Win Program This program is titled win/win because it is designated to help the employee save money and it also costs the plan less. If you use any of the hospitals in the win/win program, you will automatically be eligible for: – $500 – Out-patient Surgery – $1000 – In-patient Surgery **A check will automatically be sent to you in the mail.

Flex Program Updates The amount you flex is pre-taxed. Meaning they take the amount out of your paycheck first, then they tax you on the remaining. The tax is approximately 20%. Flex can be used for: 1. Medical 2. Dental 3. Prescriptions 4. Vision - **Exams/Glasses/Contacts 5. Babysitting Max amount you can flex: – $2500 – Single – $5000 – Married filing jointly Example: – If you flex a total of $1000/20 pays = $50 per pay period – $1000 X 20% = $200 savings The amount you flex will be available July 1, 2016 If you do not use all the amount you flexed, you can (only) roll over $500 for the next fiscal year. You will receive a flex debit card and pin in the mail for the amount you flexed. The card can only be used in the US.

CuraLinc CuraLinc in a FREE employee/immediate family assistance program. – You or your immediate family are eligible for up to six (6) free visits – They will use someone that is a provider in case you would need more than six (6) visits.

Teledoc Available if you have coverage in the US Ready to use by July 1, 2016 US Board Certified Doctors 24/7/365 You will need to set up an account to use it You can also call Teledoc for assistance registering

Sick Leave Bank Employees interested in donating: – Hourly – have at least 80 hours of annual leave accumulated You would donate 8 hrs – Salary – have at least 10 days of annual leave accumulated To qualify you need: You would donate 1 day Can be used by any employee that has: 1. Donated for that fiscal year 2. Exceeded their annual/vacation leave 3. You can have max two (2) request per fiscal year (maximum of 20 days per year).

Open Enrollment Only during the month of May you will be able to: – Make any changes to your health insurance (Medical, Dental, Vision, Mexico Only Plan, and/or US Coverage) You cannot make changes to your plan at any other time, unless it’s a qualifying event. – Qualifying events are: Marriage, Divorce, Birth of a Child, Loss of Coverage, and/or Court Ordered. – Participate in the Flex Program for the 2016/17 FY – Donate to the Sick Leave Bank ANY CHANGES YOU MAKE WILL BECOME EFFECTIVE JULY 1, 2016.
