GV Health Services GV Health is the main referral health service for the Hume region. Our services include a 24-hour Emergency Department, Surgery, Medical Services, Women’s and Children’s Services, Rehabilitation and Palliative Care, Mental Health, Outpatients, services at Tatura and Rushworth, and community-based health programs. We pride ourselves on delivering patient-centred care and we aim to enhance the patient experience through improved service access, developing partnerships, meeting growth in demand, implementing innovative service models and ensuring workforce flexibility. GV Health is focused on all stages of health care from prevention (health promotion and education), through to assessment, early intervention and treatment.

The Role The Health Independence Program Manager is an exciting newly created role arising out of a services and workforce review. The role will provide opportunities to lead service development in the subacute ambulatory setting.. GVH has decided to consolidate the HIP suite of services (SACS; HARP/Complex Care; PAC and RIR) and streamline the reporting to a designated role within the Allied Health and Ambulatory Care Division: Manager Health Independence Program. Reporting to the Divisional Director Allied Health and Ambulatory Care, this position has responsibility for the leadership and management of effective, high quality and person focussed Health Independence Program (HIP) services. The Manager Health Independence Program will support GV Health’s commitment to develop subacute services and assist in raising the profile of the organisation as a regional subacute service provider in the Hume Region. The Manager Health Independence Program will work across internal GV Health programs and services and with key external agencies to facilitate a planned and integrated approach to service delivery. The role will: Build a sustainable consolidated model for HIP service delivery, Strengthen collaborative approaches for clients with chronic and complex care conditions, Ensure HIP services work in partnership with community-based services to ensure a smooth transition and discharge process for clients moving from inpatient to community based settings, and across the care continuum, Build workforce capability across HIP, including development of a plan to guide workforce and professional development priorities.

Key Selection Criteria Essential: 1. Relevant tertiary qualifications in a health discipline, including current registration with AHPRA (as applicable) and eligibility for membership of relevant professional body. 2. Extensive experience in the healthcare setting with post-graduate qualifications in health management/relevant area of specialty or working towards same. 3. Demonstrated experience of participation in and leading multidisciplinary teams. 4. Demonstrated clinical experience and/or expert knowledge regarding evidence based practice in ambulatory care settings. 5. Demonstrated high level of analytical thinking and investigative skills involving research, benchmarking and resource identification, including innovative service delivery models. 6. Demonstrated ability to manage human resources, financial and budgetary processes. 7. Demonstrated high level negotiation and influencing skills, and the ability to engage key internal and external stakeholders. 8. Satisfactory National History Criminal Check prior to commencement of employment 9. Satisfactory Working with Children Check prior to commencement of employment (if Required) 10. Current Victorian Drivers Licence. Desirable: 1. Experience in program redesign and evaluation, process improvement and change management in the ambulatory care setting. 2. Demonstrated ability to initiate and maintain effective professional relationships across a range of regional services

Remuneration HS7 or equivalent professional classification depending upon qualifications and experience A competitive Total Remuneration Package (TRP) will be negotiated with the successful candidate.
