A Community Service Provider Dedicated to Providing Self-Directed Supports for Individuals with Intellectual Developmental Disabilities, Empowering Them to Reach Their Fullest Potential (How to address a Concern, Complaint, or Problem.)

What you can do if you have a concern or a complaint.

Talk to Staff If you have a concern or a complaint, the first thing to do is talk with a Staff member. Any Staff member will be happy to listen to your concerns and work with you to come up with a solution to the problem. If you are not happy with the solution offered, you can talk to the Adult Day Coordinator.

LEVEL 1 - INFORMAL Talk to the Adult Day Coordinator The Adult Day Coordinator will listen to your concerns and consider the solutions that have been offered by you and the Staff, then work with you to come up with other solutions to the problem. If you are not happy with the solution offered, and you need assistance, the Adult Day Coordinator will help you fill out a Level II - Request for Formal Review Form to assist you in proceeding to the next level.

LEVEL 2 – FORMAL Talk to the Program Director The Level II – Request for Formal Review Form, requesting a formal meeting is sent to the Program Director. The Program Director will meet with you as soon as possible, trying to keep within the time frame of 5 working days. The Program Director will listen to your concerns, consider the solutions that have been offered and work with you to come up with another solution to the problem. If you are not happy with the solution offered, and you need assistance, the Program Director can help you fill out a Level III - Request for Formal Review Form to assist you in proceeding to the final level.

LEVEL 3 – FORMAL Talk to the President of Carobell The LEVEL III - Request for Formal Review Form, requesting a formal meeting is sent to the President of Carobell. The President, or her designee, will meet with you as soon as possible within the time frame of 10 working days, to review your concerns and consider all offered solutions. The President, or her designee, will work with you to develop a final resolution to your problem or concern.
