Pedestrian In-Plant Vehicle (PIV)  PIV Trends  YTD US - Sentinel Events  Site Report Outs –Davison Road CCA Earl Tate & Len DotsonEarl Tate & Len Dotson –Fort Wayne Assembly Byron Walter, Santiago Martinez & Lara RyanByron Walter, Santiago Martinez & Lara Ryan  What is a Performance Standard (PS)  10 Proposed PIV Initiatives  Small Group Review Sessions  Feedback + Solutions

PIV Trends  PIV is our NUMBER ONE risk to our people  Facilities who participate in the Health & Safety Process Portal have submitted over 2,000 PIV incidents in 2013  Significant incidents: –Marion – Foot Amputation –Flint – Finger Amputation

YTD US - Sentinel Events PIV, ME, MSS & OTRV Combined = 264!

Fort Wayne Assembly and Davison Road CCA

Fort Wayne Assembly Byron Walter, Santiago Martinez & Lara Ryan

Fort Wayne Assembly Visit Cutting windows in walls to eliminate blind corners Creating one way aisles Swing gates to block the aisle for pedestrian transportation of materials

Davison Road CCA Earl Tate and Len Dotson

Davison Road CCA Visit  PIV Committee on- floor evaluations  Potential Best Practice: One driver assigned to two consecutive dock doors. This practice eliminates trucks crossing another’s path.

Some Other Sites Visited  Flint Assembly –Stop Lights –“Cattle Gate” for Maintenance –Layout Review Process  Lansing Delta Township –Benefit of being newer facility –Separation of Pedestrians and Vehicles

What is a Performance Standard?  A Performance Standard is a set of requirements based on a specific common risk  Ultimately may become a part a UAW-GM Safety System  Provides the framework for the Plan, Do, Check, Act (PDCA) continuous improvement cycle  The objective is to find cracks in the armor before someone gets hurt

Group Activity

Small Group Review Sessions  STEP 1: How would this Initiative help reduce PIV Incidents?  STEP 2: Do you see any Roadblocks that would prohibit you from implementing this initiative?  STEP 3: If Yes, what are potential solutions to eliminate the roadblock?  STEP 4: If this initiative is adopted into the performance standard, how would you do to implement it?  STEP 5: Any additional initiatives/ideas?

10 Proposed PIV Initiatives  Hazard Identification & Risk Assessment  Layout Review Process  Dock Door Loading/Unloading Sequencing  Use of Barriers  Roles & Responsibilities  Training: If we have a PIV Incident, should both parties be trained? (ME Operator and Pedestrian)  Orange Crush Zones  Pedestrian Movement of dollies, carts, material  Mobile Equipment safety devices  Engineering Controls: Intersections, aisle ways, etc.
