1 |1 | Welcome Why am I here? You are a health care worker in a primary health care facility, and are a crucial part of the Ghana Fight Against Epilepsy Initiative. You have been chosen to undergo training in the management of epilepsy, so that you will know how to recognize, treat, and follow-up people with epilepsy. Why is the Fight Against Epilepsy Initiative important? Too often, people with epilepsy are unable to access treatment even though it is effective and affordable. With treatment, 70% of people with epilepsy can lead normal lives. The Initiative seeks to help people access treatment, and one of the ways is to train health care workers in epilepsy management.

2 |2 | Why is my role important? As a health care worker, you are the key person that directly interacts with patients with epilepsy. You are the one who has the chance to make their lives better.

3 |3 | Monitoring and Evaluation Forms 3 forms New Patient Form Follow-Up Patient Form Facility Monthly Form The data collected on these forms provides the most important information about the progress of the Epilepsy Initiative. Your role as a health care provider and data collector is very important for the success of the Epilepsy Initiative.

4 |4 | Form 01: New Patient Form Complete at first contact only Fill in completely, no blank spaces Follow instructions carefully Don’t forget! Enter information into the New Patient Information Sheet

5 |5 | New Patient Data Sheet To be filled out after every new patient Fill out one row for each new patient Completed Information Sheet will be collected by District Coordinator each month New Information Sheet will be provided each month

6 |6 | Form 02: Follow-Up Patient Form To be completed for all follow-up consultations Fill in completely, no blank spaces Follow instructions carefully Don’t forget! Enter information into the Follow- Up Patient Data Sheet

7 |7 | Follow-Up Patient Data Sheet To be filled out after every follow-up patient Fill out one row for each new patient Completed Information Sheet will be collected by District Coordinator each month New Information Sheet will be provided each month

8 |8 | Form 03: Facility Monthly Report Form To be completed once per month by identified facility staff member Fill in completely, no blank spaces Will be collected by District Coordinator once per month, and sent to the National Focal Point for the Epilepsy Initiative

9 |9 | Final Thoughts Remember! As a health care provider you are a crucial part of this initiative to bring epilepsy care to people with epilepsy. Through this training you have the skills to recognize, treat and follow-up people with epilepsy, and have the chance to improve many people’s lives. Representatives from the World Health Organization, Ministry of Health, and other stakeholders thank you for your participation!
