Rwanda South Africa Burundi Uganda Ethiopia Cambodia Vietnam Lesotho Surinam Zimbabwe Zambia Kiribati Nepal International focus: funding and drug costs  However: non-existing delivery system  Didactic training programs BUT: “theory” is not “skills” ICEHA: Pioneer: New concept for capacity building: mentoring  Supply volunteer expert HCWs to developing countries for rapid skill transfer using a method of mentoring(as a follow-on to didactic training)  Mentoring focus: practical clinical skills and operational systems  Western HCWs wanted to volunteer  Developing countries requested the clinical mentors: Hospitals, clinics, NGOs, MOH’s Initial countries: Cambodia and Vietnam 2001: HEALTHCARE CAPACITY FOR HIV

Rwanda South Africa Burundi Uganda Ethiopia Cambodia Vietnam Lesotho Surinam Zimbabwe Zambia Kiribati Nepal Internationally: Very different picture  International recognition: didactic is not enough  WHO guidelines  Clinical mentoring included in ART scale-up programs ICEHA: remains the only global clinical mentoring organisation  >750 volunteer clinical mentors  (50%) Doctors and clinicians; (50%) nurses, social workers, lab technicians  sourced from 14 countries in the West + Diaspora  Typically: 10+ years of clinical experience  Minimum time commitment: 6 weeks  13 developing countries and growing list of requests for mentors 2008: HEALTHCARE CAPACITY FOR HIV

Rwanda South Africa Burundi Uganda Ethiopia Cambodia Vietnam Lesotho Surinam Zimbabwe Zambia Kiribati Nepal developing countries in the driver’s seat: local guidelines, local resources, local requests continuous clinical mentoring 3 – 4 months/site until “mentored out” customised mentoring / clinical site intense screening and pre-deployment preparation of volunteer clinical mentors in-country preparation of local HCWs in-country pre-and post-deployment briefings for clinical mentors micro-management of mentors while in-country strong quality control with local partner / MOH co-investment When does clinical mentoring work?

Rwanda South Africa Burundi Uganda Ethiopia Cambodia Vietnam Lesotho Surinam Zimbabwe Zambia Kiribati Nepal What is the impact? IMMEDIATE AND SUSTAINABLE RESULTS: Clinical skills for comprehensive care Operational systems for comprehensive care Number of patients in HIV care and on ART Retention of local HCWs Positive spill-over effect on other programs
