Southern Health NHS Foundation Trust – Adult Mental Health Directorate Productive Steering Group in place meeting monthly. Emphasis on setting up framework and sustainability. Planning workshops, coaching senior leadership and demonstrating success. All areas using project plan and dashboard to maintain momentum and standardise progress reports In early stages of planning to deliver lean training through PD&T 4 Flow Built in Quality Performance Measures Problem Solving Standardisation Continuous Sustained Improvement

Improved employee morale, job satisfaction and opportunity for staff development –A 20% reduction in unplanned absences amongst staff –A 20% increase in staff satisfaction –A 70% reduction in violent and aggressive incidents Increased productivity –Efficiency savings of £5,962 per month per acute inpatient unit Development of continual improvement culture –Facilitation of PMHW & PCS packages Greater accountability, joint working and standard framework for continuous improvement within the business Improved safety as a result of streamlined environments and standard working –A 40% reduction in medication errors Processes that are consistent, efficient and effective at all levels –A 15% increase in value added activity and direct care time Increased customer satisfaction and experience –A 15% increase in patient satisfaction Southern Health NHS Foundation Trust – Adult Mental Health Directorate Results & Plan

Southern Health NHS Foundation Trust - North Hants Ward Performance board in place. First module 60% completed awaiting integration of ward performance meetings into ward routine. Baseline activity sampling completed. E.G Time in motion study identifies possible savings of: - 1,425,899 steps - 93 hours (13 clinical shifts) Nursing staff leading on the re-organisation of key ward areas into lean cells ensuring environment supports quality patient interaction Two productivity workshops held with 3 rd due before end of October. Heavy involvement of MDT and workshop held for medical team. Performance board highlights key areas of waste (food, medications). Transparent means of communicating ward SMART goals and targets

Southern Health NHS Foundation Trust - West - Winsor Routine ward performance management meetings in place during steering group meetings. Winsor Ward have observed over a 6 month period: - An 80% reduction in violence and aggression - An increase of 18% in patient satisfaction -A reduction in medication errors by 75% Redesign of handover system has reduced time spent in handover but also reduced errors and omissions. Excellent feedback from divisional lead nurse. Charge nurses leading observations, patient wellbeing, patient status, admissions and discharges, medications and ward environment modules. Sustainable framework in place. Completed modules being worked through again to relentlessly remove waste from the workplace. Overhaul of activities and therapies system reduces negative satisfaction by 47%

Southern Health NHS Foundation Trust - West - Melbury Streamlined clinical stores room has reduced inventory by 48 boxes thereby minimizing waste associated with overwork and time spent looking for stores. (motion) OT leading on increasing stakeholder feedback to clinical teams. Sustained improvements in clinic room organisation. OT team completed patient status at a glance module having redesigned status board, improving confidentiality and reducing interruptions. Performance board meetings to be integrated into ward routine. Next steps are to ensure framework in place for sustainable improvements

Southern Health NHS Foundation Trust - South Handover module 40% complete awaiting review of progress and planned redesign of system. Already identified vast areas of savings and increases in quality. Ward performance board designed. Trinity have already observed: - Significant reductions in self harm - Significant reductions in AWOLS Ward reception and office in initial stages of transformation into lean cell. Work being led by support workers Charge nurses leading on medications module. In initial stages. Steering group in place with representation by service users and carers. Baseline activity sampling and further productivity workshops planned in

Southern Health NHS Foundation Trust - East – Meadows & Elmleigh Meadows - Review of progress in place. Steering group continues to oversee progress through modules. 45% Programme completion. Elmleigh – Focus on consolidating work on foundation modules. Sustained work in Well Organised Ward module. Patient Status board redesigned to reduce interruptions and time spent looking for information. 1 2

Southern Health NHS Foundation Trust – Adult Mental Health Next Steps In partnership with PD&T develop centralised training package on Productives and Lean Theory Options paper to OMB in September to plan end of October/February. Productive Web – Sharing best practice and successes, challenges Presenting successes to SHA and providing training on PMHW rollout during SHA sharing event Further emphasis on continuation planning. Development of local leadership and roles and responsibilities Continued roll-out of modules throughout inpatient services Implementation of visit pyramid and structured shop floor visits Centralised reporting of measures and progress against SMART targets
