© Homewood Health™ Confidential & Proprietary 28 Regulatory Oversight Schedule 1 Facility under the Ontario Mental Health Act Private Hospital Service Accountability Agreement (PHSAA) – Provides service volumes for inpatient mental health days and ambulatory care visits for Ontario patients Multi-Sector Service Accountability Agreement (MSAA)  Provides service volumes for face-to-face visits, telephone contacts, and the number of individuals served in Ontario We submit annual budget, quarterly and year-end reports through SRI (Self-Reporting Initiative) for both inpatient and community services Management Information Systems (MIS) reports are submitted quarterly and at year end to the Ministry of Health and Long-term Care Health Data Branch We report to the Ontario Mental Health Reporting System (OMHRS)

© Homewood Health™ Confidential & Proprietary 29 Operating Budget and Funding $67.7 million total operating budget Less than half of our budget is funded by the LHIN/Ministry of Health and the balance is largely made up by out-of-province admissions and accommodation revenue Over the last 14 years, funding provided by the LHIN has declined from 58% to 45% of total revenue (no increase to funding for the last 3 years) Yet, over that same period the capacity dedicated to Ontario patient days has ranged from 85% to 92%, of our total capacity in addition to providing ambulatory services Homewood’s PHSAA patient day commitment for 2014 – 2015 is 87,663 LHIN-weighted patient days and 9,500 ambulatory care visits Homewood receives no capital funding from the Ministry of Health
