The submitter will be contacted prior to any photo use.  On the form provided, place a safety related photo and short memo explaining why safety is important to you Example: a vehicle accident that totaled the vehicle but you/or a family member survived with minimal bodily injury.  All submissions must be turned into your local safety department by Friday, October 10, 2014  Safety department must forward all submissions to the CHR by Friday, October 17 th  The CHR will select 5 entries to be posted on the UAW-GM JAS website  The CHR will notify recipients if their photo entry is selected UAW-GM SAFETY WEEK Guidelines for Safety Photo Entries
