Department or Division3 Concerns Raised Around Presentation in School Self harm (marks on arm) Mood scales completed by Emotional Well-Being Worker in school which indicated low mood Referral made to central team following discussion and agreement with young person and parents SDQ and YP Core completed STAR Risk Assessment carried out

Department or Division4 Issues from Assessment Pro-social skills High level of emotional distress Stress No acknowledged thoughts of self- harm or suicidal ideation

Department or Division8 Intervention Central team assisted school to complete a CAF and organise a Child in Need (CIN) meeting in order to formulate a co-ordinated package of support and to ensure we dealt with john’s needs sensitively The following agencies were involved in the CIN meeting where information was gathered and a plan formulated: Adult mental health School Housing Parents Young person 2 B me Adult Social Care

Department or Division9 Intervention Direct work with John’s parents which promoted their understanding of John’s needs CBT based interventions with John Practical self-esteem boosting work Regular reviews of the CAF through CIN meetings School updated the CAF regularly with all agencies, John and his family.
